High School Enrollment Forms

Welcome to Elkins High School!

Please bring the following when enrolling:

1. Birth Certificate
2. Immunization Record
3. Social Security Number
4. Proof of Residency (electric bill, rental agreement, etc.)
5. Forms shown below (fill out ahead of time and print or forms can be filled out in the office)
